Breast Augmentation (Mammoplasty)
What is Breast Augmentation?
Breast augmentation (mammoplasty) has become one of the most common plastic surgery procedures for women, regardless of their age. According to the American Society of Plastic Surgeons, more than 300,000 breast augmentation procedures are conducted annually.
This cosmetic procedure enables a patient to have their breasts adjusted, renewed or recreated to suit their lifestyle and needs.
Dr. Peter Chang, founder of Timeless Plastic Surgery, began performing breast augmentations and other breast-related surgeries more than 15 years ago. His continual advancements in the art of plastic surgery continues to garner him high praise from patients and the medical community.
What Separates Dr. Chang from Other Plastic Surgeons?
Dr. Chang conducts three pre-operative visits with each patient in order to discover the wants and needs of the patient, discuss their expectations, and provide in-depth information about the procedure. During the consultation, he ascertains the necessary health and medical information to avoid any possible complications that may stem from allergies, prior surgeries, or health problems.
“Many surgeons see a patient once before surgery, maybe twice. I like to see patients three times before surgery: consultation, pre-op visit, and the morning of surgery,” Dr. Chang says.
He has established several surgical techniques that have allowed him to avoid what he considers common complications with breast augmentation surgeries. These complications can include capsular contracture, double-bubble, and rippling. By spending nearly two decades studying in the medical field, earning two doctorate degrees from Ivy League institutions, Dr. Chang has an exceptional understanding of the human body.
This understanding of the human anatomy provides him unassailable insight with each patient on what their body and skin type can accept. Dr. Chang physically measures the breasts multiple times, utilizing a specific technique, to help secure the right size and shape for the patient.
Not only is the physical and surgical aspect of a breast augmentation conducted with great care, but also the emotional aspect. Every patient is seen through the process by the same staff members so there are no new faces or surprises when coming in for surgery.

Implant and Cup Sizes
The range of breast implant sizes are based solely on the patient’s body type and how wide, long, and thick the breast tissue is. Dr. Chang warns patients not to deviate too far from the natural cup size. For many patients, there is uncertainty about the difference between implant size and cup size. During the patient’s consultation, they are provided information on the range of implant selections, from silicone and saline to smooth-round and teardrop, as well as how different manufacturers differ in their implant sizes.
The focus behind choosing the correct implant size and cup size is to make sure the breasts remain proportionate to the rest of the body and that they provide a natural appearance. Another reason is because not everybody is the same, therefore the laxity of the skin differs from patient to patient. Dr. Chang wants to ensure that every patient receives the look that is best for their body with the fewest possibilities for complications.
Saline, Silicone and Implant Types
Since this procedure is conducted so often, having a board certified plastic surgeon who is experienced and has a successful track record of performing breast implant operations is imperative.
There are two types of breast implants: silicone and saline. A silicone implant is filled with silicone gel, while the saline implant is filled with salt water. Both require different sizes of incisions and both vary in their feel and appearance. The two are not without their specific benefits and risks.
When it comes to size and shape, Dr. Chang prefers the use of the smooth-round breast implant for each of his patients, simply because this type of implant has been tried and tested over numerous years and continues to provide high quality results.
Breast Implant Placement Options and Techniques
There are three placement options conducted by Dr. Chang when it comes to placing the implant inside the breasts: through the armpit, underneath the breast (inframammary fold incision), and under the nipple. It should be noted that with every breast surgery, there is a chance for nipple sensation change. The armpit placement avoids scarring on the breasts and is used on smaller cup increases. It cannot be conducted with drooping breasts.
Inframammary fold incision is the most common implant placement for breast augmentation and the scar is easily hidden with clothing and bras. It avoids nipple nerves, significantly lowering the chances of nipple sensation changes. It also does not obstruct breastfeeding.
The nipple placement makes an incision along the brown tissue and the breast tissue, giving an upward curved line. This option, however, has a higher chance of nipple sensation changes and could obstruct breastfeeding.
Preoperative Preparation for Breast Augmentation
For every surgery, patients must abide by various guidelines provided by Dr. Chang. This is done in order to protect the patient from potential complications during and after surgery.
He meets with every patient three times before their breast augmentation surgery, with the final meeting being the morning of the operation. This is done to ensure there are no last minute changes and that he and the patient are on the same page when it comes to performing the operation and ascertaining certain results.
The patient’s overall health, cleanliness, and current medicinal intake must be up to the Timeless Plastic Surgery standards.
Post-Operative Preparation, Care, Risks and Recovery Timeline
With any surgery, there are always risks. The risks that stem from breast augmentation surgery include swelling, bleeding, excess fluid, bruising, deflating or rupturing breast implants, and capsular contracture.
Dr. Chang and the staff at Timeless Plastic Surgery go above and beyond the common surgical routine to lessen the chances of possible complications and provide every patient with the pertinent information to have a truly successful outcome to their breast augmentation procedure.
Patients are provided with pain medication and antibiotics to fight off possible infections. Each patient is informed of the recovery timeline and how many days or weeks it will take to begin resuming regular activities, such as work, exercise, and heavy lifting. The timeline itemizes certain recovery goals each patient should keep in mind, such as the reduction of swelling or when the breasts settle into place.
For postoperative care, Timeless Plastic Surgery provides several items to assist in the healing process, including a surgical bra and antibiotic ointment. In order to secure a full and perfect recovery and to ensure great results, the patient must be willing to follow particular instructions provided by Dr. Chang. These results will be managed by regularly scheduled visits to Timeless Plastic Surgery as part of the postoperative protocol. For the lifetime of the patient, there are annual visits.
Replacing Breast Implants
Dr. Chang recommends having the implants replaced every 10 to 15 years. Some breast implants have been known to last the lifetime of the patient (20 to 30 years), but this should not be considered a common occurrence. He says that a young patient may wind up replacing their breast implants two or three times during their lifetime.
“It is not a one-and-done thing; there’s a maintenance process. I wouldn’t go past 20 years without a change. But as long as the implants are healthy, they don’t need to be changed.” – Dr Peter Chang
